Your First Massage Appointment

Please take a moment to download and fill out the following forms and bring them to your first massage appointment at the office.  Please allow an additional 10 to 15 minutes for your first massage session.  This extra time is for your therapist to review your health history and to discuss the best course of therapy for you.

Your First Acupuncture Session
